Licensed Practical Nurse (LPN)
Job Description
Licensed Practical Nurse (LPN) – Residential Group Home
Location: Woonsocket, RI
Schedule: Full-Time, Flexible Scheduling (Primarily Day/Evening Coverage)
Are you an LPN who values meaningful, relationship-based care? Join a dedicated residential team supporting adults with developmental disabilities and medical needs in a warm, home-like environment. This role offers the chance to provide hands-on nursing care while being an integral part of residents’ daily lives.
If you’re looking for a slower-paced setting than acute care—where you can truly get to know the people you support—this could be the right fit.
What You’ll Do:
- Deliver direct nursing care in a residential group home setting
- Administer medications, treatments, and monitor residents’ health status
- Support individuals with activities of daily living alongside direct care staff
- Maintain accurate health records and documentation
- Communicate with physicians, families, and interdisciplinary team members
- Respond to medical concerns and ensure a safe, supportive home environment
- Promote independence, dignity, and quality of life for each resident
Why This Role Stands Out:
- Work in a comfortable, home-based setting rather than an institutional environment
- Build meaningful, long-term relationships with residents
- Be part of a supportive, mission-driven team
- Make a direct, daily impact on individuals’ lives
If you’re ready to bring your nursing skills into a setting where care is personal and purpose-driven, we encourage you to apply.
Job Requirements
What We’re Looking For:
- Active LPN license in Rhode Island
- Experience with developmental disabilities, long-term care, or community-based settings preferred
- Strong observational and communication skills
- Ability to work collaboratively with residential staff
- Compassionate, patient, and person-centered approach to care
