Job Description

We’re hiring a Field Response Clinician to deliver assessment, intervention, and care coordination services in community-based settings. This role is ideal for clinicians who prefer working in the field, value autonomy, and want to support individuals and families by meeting them where they are—literally and clinically. You’ll work across hospitals, clinics, schools, homes, and partner agencies, providing timely evaluations, short-term interventions, and thoughtful recommendations that connect people to the right level of care.

What You’ll Do

  • Conduct comprehensive clinical assessments, including risk screening and level-of-care recommendations.

  • Provide short-term, solution-focused interventions in community and facility-based settings.

  • Respond to field-based requests from hospitals, schools, families, and partner agencies.

  • Coordinate care with internal teams and external partners, including hospitals, DMH, DSS, schools, and community providers.

  • Provide follow-up and case management support as needed after initial evaluations.

  • Facilitate referrals to inpatient, respite, detox, and outpatient services when appropriate.

  • Support phone-based triage and referral coordination as part of the team rotation.

  • Engage individuals, families, and referral sources with professionalism, empathy, and respect.

Job Requirements

  • Master’s degree or higher in Social Work, Counseling, Psychology, or a related clinical field.
  • Licensure required (LCSW, LICSW, or LMHC within six months of eligibility).
  • 2–3 years of clinical experience preferred; community-based or field work a plus.
  • Strong clinical judgment and comfort making independent decisions in dynamic settings.
  • Ability to manage time, documentation, and follow-up in a mobile role.
  • Valid driver’s license required.
